We are developing a list of questions that will be emailed to customers to respond online after each time their receive service. What is the maximum number of questions that should be on this card? Can you provide some resources/articles to review?

There isn't really a hard and fast rule to the number of questions. However, the more questions you ask, the higher your drop-out rate will be.
